📍 Venue: Camp 4 Daycare Center Area
📋 Please bring the following documents (with photocopies):
✅ Birth Certificate of all family members
✅ Marriage Contract (for married members)
✅ Death Certificate (if applicable)
✅ PhilSys ID or proof of PhilSys registration for all family members
✅ School Attendance Certificate, Valid School ID, or Report Card
✅ Health Attendance Certificate (for pregnant women and children aged 0–5 years old)
✅ PWD ID (if applicable)
✅ Medical Certificate (for family members with existing medical conditions)
